To became a contract carrier for Multi Logistics Solutions, Inc. we will need the following information completed and returned in full:
1.
Current Insurance Certificate ( stating cargo coverage, auto liability coverage and list Multi Logistics Solutions as certificate holder).
2.
Copy of ICC Authority
3.
DOT Safety Rating Letter
4.
Signed Contract (all 6 pages)
5.
Carrier Profile (all information completed)
6.
Federal ID number ( W-9 Form with 1 number ie, Employer Is or SSN)
7.
Miscellaneous
Attached you will find all necessary forms to be completed.
ALL 6 PAGES OF THE CONTRACT MUST BE FILLED OUT AND RETURNED IN ORDER TO COMPLETE SETUP RPOCESS.
